I'm so piggin mad at myself

Hi all,
First time I've posted on here, but I feel like I'm at my wits end and I don't know what to do.
I realised that I want to lose weight June/July time after a visit to London, I hated the way that I looked and felt.
I went to the doctors and explained everything and she was really really nice. Long story short but in 3 months I lost 21lb's,which I was really really happy with. I'm now at the point that I put 4lb on in month 4 and my weigh in is Friday this week, and I know that it's not going to be good, def no weight loss the best I can hope for is a maintain.
Truth is I have totally fallen off the wagon but I still see whee I want to be, I just keep letting myself down. I'm so pissed at myself.
I set myself a target for a size smaller jeans for Xmas but that suddenly feels like 10 million miles away.
I'm worried about the appt, as I don't want to lose the support from the doctors as I worry they will do that if I don't show progress. I'm also worried that I can't get back on track.
Can someone please help me - give me some advise or something. I feel like crying.

Oh honey....
Deep breath. Then perspective, perspective. Yes you gained 4lbs but you lost 21 so that's a NET loss of 17lbs. Which is amazing! Everyone falls off the wagon, everyone. Log it and move on - you're starting from a good place here, you've lost over a stone!
You CAN get back on track. Just take it a meal at a time.
ALSO give yourself permission not to be perfect.

You can TOTALLY do this! Like the others said, take it one meal, one workout and one day at a time! Let's not forget that it's extra difficult to go it alone, having someone or a group of people to support you and keep you accountable can really work! Feel free to add me as a friend!0

I fell off too. I gave up on myself for 6 days (which doesn't sound like much, but it was enough to gain back three pounds.) Then I looked at myself in the mirror. Naked. I thought about my kids and my husband, and what kind of illnesses await me if I don't get to a healthier weight. Maybe you just need more motivation than looking good or fitting into that pair of jeans? Don't give up on yourself. YOU ARE WORTH IT!!!!! Also, feel free to add me, if you want a buddy to help keep you accountable for keeping up with your diet/exercises. Today's as good a day as any to get back on the wagon0

Do you track your calories or just eat 'sensibly/diet food'? I used to take the sensible eating approach but then I'd feel that if I'd eaten a 'forbidden' food, I'd blown it for that day (and so defiantly ate more)...a couple of days like that and I'd written off the week. Once I started tracking calories, I found that no food was forbidden (I eat chocolate and/or cake most days) and I could choose what I eat and when as long as I hit my calorie goal. Takes a bit of trial and error to find what keeps you satisfied both taste and fullness wise but it works! Then if you have an unplanned calorie loading (eg treats at work) quickly figure how bad the damage was and work on limiting it with low calorie food options for the rest of the day or adding some cardio exercise in to counteract. My worst mistake with dieting was thinking in terms of days and weeks...that diet that's going to start (or getting back on the wagon) tomorrow or next Monday leads to binge eating until it does (in my experience) and the resulting more fat to get rid of when you do start.
