Help on starting MFP?

I downloaded this and tried it a while ago, but never did much with it.
No matter what the type of plan is, I always end up giving up. It seems like counting calories is always the most difficult for me, because once I'm stuck in a situation where I can't get an exact count (eating out at a restaurant or something not listed on MFP or anywhere else, for example), I feel like I've messed up and because I can't determine how I did for the day. Then it slowly (or not so slowly) unravels from there and I end up just giving up overall.
This is my most common experience, followed by just a lack of motivation. It's so hard to stick with it, and I want to become healthy so badly, but sometimes it feels like I'm reaching for the stars.
Can anyone relate to this? Does anyone else have any insight?
I have always been on the heavier side. Then I started college and gained nearly 150 pounds! I've since lost 40 pounds, but it didn't require much effort because I was just SO out of shape. I've been at the same weight for nearly 1.5 years now. I have 145 pounds to lose until I get to a "healthy" weight, and I could use all the support that I can get!
