Diastasis Recti

Has anyone dealt with this before? I just learned that I have this and I am a little discouraged. I am 28 years old, have 2 boys ages 7 and 4, I am 5'4" and 114 pounds. I have a small frame, and I'm pretty little, but have had what I thought was a typical "mommy tummy" consisting of fat. I have done several Jillian Michaels' workout DVDs and have seen only minimal changes in my abdomen. As a family, we are pretty healthy eaters.
I am almost into month 2 of Jillian Michaels' Body Revolution and I am wondering if I can work on the Diastasis while continuing this workout. I have heard that a lot of the regular crunches and planks can actually make this condition worse. Ideas? Has anyone dealt with this before? Is there a good program out there that can help me?
I appreciate any info you can offer
