Lifting Question - Calculating weight

I recently started NROLFW and I am a little confused as to how I should be tracking the weight I am lifting. For example, on the Dumbbell Shoulder Press I use two 15lbs dumbbells and I track this as 15lbs. Where I am confused is when using a barbell. For squats if I am using a 45lbs barbell and add one 10lbs plate, and one 5lbs plate to each side do I count that as 75lbs (bar+30) or 60lbs (bar+15)?
Also, as the reps decrease in the program the weight should increase; what if I am unable to increase the weight to the next increment? Should I stay with the higher reps even though the workout says to do less?
