Need a Iphone app to guide me through workout?!

I realized last night at gym the biggest thing is I get lost at the gym. I know I am starting with medicine ball toss ups and dumbbell squats. Then I am done and kinda forget what's next.
I don't want to carry around a sheet of paper or notebook.
Are there any apps where you can enter all your exercises, how many to do, and weight. Then the phone will show the exercise and when your done you hit done and the next exercise pops up?
One that times your or motivates you in headphones would be even better.
Nike Training Club does it but nothing with weights - just bodyweight.

I like the "iFitness" app. It's got several workout plans to get you started. Lots of exercises to peruse. And I like how it lets me log my routine!0

HOT5 is great, you download the 5 minute videos you choose and when you do them, it posts right to your account here if you link them.
I have done a couple of them, I like being able to work out 5 minutes at a time as I choose0
