Not sure if I'm doing this right (eating enough?)

So, I think I'm pretty good with the *sources* of all my food--it's mostly all natural and home-cooked and healthy and all that stuff--but I have no idea if I'm eating the right amount of everything.

My goal is to gain muscle without gaining fat, hopefully staying around my same weight (165ish lbs). So I've put MFP on to a maintenance plan, and I'm lifting heavy 2-3 days a week, cardio another day (sometimes). Been doing this for about 6+ months now, but nothing is changing! I'm actually gaining fat in my abdominal area (though losing it in my arms and legs).

I think for some reason my food might be the culprit. Any kind souls want to look into my diary and help me out? I'm getting 30-40% protein most days, and eating around my maintenance calorie number, so I could just use a second (or third, or fourth, or fifth) opinion.
