Raw meat or cooked meat?

I've been weighing my meat out for awhile now and I always weigh it cooked, but it occurred to me that the weight is probably for raw. Most entries and labels don't specify if it's for raw or cooked so I'm assuming it's all for raw values? How do you log meat? Why is this so hard for me to figure out?

Generally it's raw. Pretty sure that's how the FDA and restaurants calculate it.
Edit: Actually think that's USDA. oops.
