Gym routine after back surgery

In February I had back surgery for a major slipped disc, I had previously had almost a year of severe pain that had restricted me from even walking normally.
I am now in full recovery and able to move freely but am obviously apprehensive about doing anything that could cause the injury to recur or set my recovery backwards.

I'd like a mix of cardio and weights but my fitness is seriously low and I can't jog for more than about 30 seconds. I'd really like some help on which machines to use, any sports that would be good, routines or classes you've found that would be good?
