I need help with motivation.

Hello everyone!
I want to become healthy & happy but being 5' 3"and 213 lbs is not working for me. I get motivation for about a week to workout and then it just disappears and I have to restart. I feel like even when I am eating healthy, that I am still gaining weight. I want to get back down to 125 lbs and a size 4. I just don't know what I am supposed to do.
Should I get a gym membership to keep me motivated?
What should I eat?
How long should I work out?
If anyone could help me, that would be awesome! :)
