Just starting out, had tricky starts before so i am worried.

Im 18 now and need to lose 4 stone at least i have tried so hard in the past once had a dietitian who i had regular meetings with she gave me a meal plan and had to eat only the foods on that and no more so i did that for 7 months and went the gym 4 times a week for about 1-2 hours i thought that whole plan would work but i put on weight so i thought it would be muscle building but stopped after 7 months because i put on 2 stone making me the weight i am now. I really need to lose weight because its knocking my confidence i suffor with a mental illness so my weight isnt exactly helping me improve my feelings.

Calorie deficit. Meet your macros. Weigh/measure all your food using a food scale/measuring cups & spoons. Patience.
This link is full of helpful information. Read it all and give yourself a head start on doing this correctly. You're young. Do it right the first time and you won't have to do it again.
