Kind of New

So I have been on here for about 2 months, the first month was great and I lost about 8 pounds. Well I fell off of the wagon for about 2 weeks and gained 6 pounds back. I exercise regularly but i am not focused. I NEED FOCUS. Plus I don't eat well. I eat what I think is healthy and beneficial and most times I make ok decisions. but sometimes, I don't or I snack like crazy at night. Advice please.....
