Where to begin?

Hi all! I have been on my weight loss journey for what seems like years. I have just recently gone through a full year's program with a weight management clinic and have had some success there. Granted not as much as I wanted initially but that was due to me not putting in the effort required. Now I am realizing that I want to be able to run around and play with my 1 year old nephew but I just don't know where to start. I have begun tracking my food and water intact. I have also begun participating in physical challenges at work which means wearing a pedometer and tracking my steps. That is all positive for me and I am very excited for that! But I also look at some of the snacking that I am doing and it is not the healthy foods. Granted I have cut out a bit of the "junk" food but it is hard. Are there any suggestions on where to start, or where to go from here?
