How to get my drive back?

I just can't seem to find the drive I once had. First a little history of my weight issus.

In 1997, I had a Gastric Bypass weighing in a 393. In a year I lost about 150ish lbs, but never did I exercise or eat well. I simply lost the weight due to fact I couldn't eat much. Over time, the weight came back on.

Fast forward to 2011. Went to a Drs. appointment, stepped on the scale and weighed in at 317. After realizing the direction was heading, something clicked. I cleaned the house of my junk food stash and joined a gym that night.

By Christmas of 2012 I was down to 215. I was the lowest I had been since I was probably 12. I'm 47 now so it had been awhile! I was determined that in 2013 I was going to hit Onederland, my final goal and never look back.

Well, it's now July 2014 and I've gained about 20 lbs since. I did manage to get down to 215 again, but gained again. Basically, for a year and 1/2 I've yo-yo'ed. I can't say I'm totally disappointed in myself though. In my past, I've done many diets where I've lost weight, but immediatly gained the weight back and more. I've kept off more than 80+ lbs but I struggle to lose what I did and hit my goals. I'm afraid of burning out and gaining back more weight before I realize it!

I guess what I'm looking for is my drive back. I don't want to fail this and be over 300 again. I have Rheumatoid Arthritis and my body can't withstand the pain of the lbs.

For those who have lost a lot of weight, are in the process and those who hit their goal, what kept you going? If you ended up stalling like myself, how did you pick yourself up again and move on. I know most of this is mental, but....

How did you keep up your drive?
