Transition from Medifast

I have been doing Medifast for about 2 months and have lost 22 pounds. It was great for a kick start but it is not sustainable long terms. I have about 45 lbs still to lose and am planning to transition soon to a lower carb eating plan (40-50 net carbs per day).
Has anyone successfully transitioned from Medifast to another plan? If so, which plan did you go to? Did your weight plateau for a while or did you keep losing weight? Trying to figure out how to do this while continuing to lose weight at the same time...
