How do you get back on the wagon?

I've been on many diets. Some last a few weeks, some last a few months. But I always fall back to my old habits (or about 90% of them) eventually.
And then I feel stuck. I might get back into the groove for one day, but then doing it again just doesn't happen. I seem to forget how I managed stress and overwork and not-enough-to-time-to-plan-my-meals. Some part of me feels like I just need to get shocked back into it, because pure will just doesn't cut it.
And I always find a way to get back into it. Sometimes it can take weeks, and sometimes it can take months. I try to remember what got me started in the first place, and how can I do that again. But I can never remember. What usually happens is that I get a new motivation that I never had before. It's kinda like my old motivations expire and have to be replaced.
So, while I'm trying to figure myself out, I'm curious about you. When you fall off the wagon for more than a few days, how do you get back on?
I suspect somebody is going to come on here and share how they MAINTAIN their motivation so they DON'T fall off in the first place. I really don't want to hear from these people. If that's you're story, start a thread and share it there.
I also don't want to hear people regurgitating some article or study they read. I really don't care what the "experts" have to say on this matter. If that's what I wanted I would Google it. I only want to hear about personal experience.
Let me be clear: I only want to hear from people who, like me, fall off the wagon and then find a way to get back on it. This is my pattern, this is what I'm up against. If this is not your pattern, than you have nothing to offer on this thread.
