Long term maintenance

"Obesity is difficult to treat and has a high relapse rate. Greater than 95% of those who lose weight regain the weight within five years" (Quote from http://www.medicinenet.com/obesity_weight_loss/article.htm) But I have heard this statistic over and over again from many sources.

My question is for those of you who have lost and kept if off for years. What makes you part of the 5%? How do you do it? Is there a secret opposed to the obvious food and exercise?
