NHS Waiting Lists

I have changed my name deed poll, i've changed everything to my new name except I havent told the NHS/GP. I'm worried that changing my name might affect my position on a waiting list, I had to wait 6 months just to be seen to see if i could go on the waiting list, im on it now and would like to stay on it.
Problem is all my I.D is in my new name and i almost signed my new name on a permission slip the other day, I must of looked stupid in the chemist too when she asked my name and i paused to think about it.
I hate my old name, thats why i changed it by deed poll. I would love to see the back of it once and for all.

Has anyone changed the name on their medical records? Was it easy to do? Did it affect any appointments or waiting?

Any advice would be great, thanks.
