Eating out while eating clean?

I'd never heard of "eating clean" until I started MFP, and I'm intrigued. The health benefits seem to be endless and there's the added reassurance that everything you're putting into your body is wholesome and natural. So I'm thinking seriously about starting to eat clean, but I won't take it so far as to become a vegan or vegetarian - just fish and chicken though, I think - but I still have a couple questions for those who have been eating this way for a while.

First, did you stop eating processed foods/fast foods cold turkey? Or was it a gradual thing, like dwindling down to those kinds of foods three times, then two times, then once weekly? Did and do you have digestive or tummy troubles if you eat clean for a while and then eat something processed?
Secondly, what about restaraunts? I eat out a lot with family and friends, and while I realize that it's not the healthiest thing, have you found clean foods that are doable at restaraunts?

Sorry for all the questions...just trying to get a better understanding and a little advice!
