OK, I guess I better introduce myself!
First of all, I want to say that the myfitnesspal iPhone app is like a godsend. It has never been so easy to be conscious about calories, fat content, carbs and everything else. It just amazes me that everything I eat is already in there!
I was always a thin kid, until middle school, when I began spending more time at my grandparent's house after school. The combination of heaping portions of home-cooked food and desserts that never seemed to end... well, it wasn't good for my weight!
In my last year of high school, I managed to lose enough weight to be at my ideal BMI. It was amazing-- I was thin, I had a better self image, and overall, I just felt better.
As time went by, though, I gained it all back. I won't forget the day I moved home from Florida, and I visited my grandmother. The first words out of her mouth were, "You got FAT!" Wow.
So now, I'm out for a complete lifestyle change. Better foods, better habits, more exercise. So far the hardest thing I have to deal with is in the evenings when I'm watching TV, I want to reach for something to snack on. I'm not hungry per se, but it's more of a craving that comes from habit. Once I retrain myself, I hope, I'll find the evenings easier to get through. For now, I'm buying healthier things for when I feel like I have to snack-- pears, apples and carrots-- and hopefully with a little support, I'll be feeling better overall eventually.

I'm in the same boat. I have always had issues with my weight and 8 years ago lost 64 lbs doing the Atkins diet. Obviously it was very unhealthy and when my wife got pregnant I gained the weight back and then some. But this is as you said a lifestyle change...its not a diet or a phase. This time its for life. Evening TV time can be rough. On those difficult nights I find a cup of flavorful tea helps a lot. If I still need something, I might eat a 100 calorie pack. The flavors in the tea plus the volume of the liquid are a win/win.
Or head out for a walk. You'll feel so good about moving that you'll want to stay away from snacks.
