How do you break out of a plateau?

I wasn't sure where to put this post, so I stuck it here. Now, to tell you a little about my plateau - I lost @ 30 lbs with diet and exercise. I ended up losing my gallbladder due to health reasons and had to stop the exercise. I weighed 172. I had some high stress (my mom was terminal and passed away) and here I am. At @ 176. I had my surgery several years ago, and have yo-yoed but now I am Really ready to move on. Any ideas on a re-start out there?
And I am steadily working on making my diet better. I still need help, but am doing ok.
Exercise is also getting better - biggest loser kinect Ultimate w/o is awesome!
That being said, I still weigh in the 170's, so what do you do for a plateau?

    Need to evaluate what you've been doing. Your diary is set to private, so we will have limited ability to give any advice without making assumptions.

    what diary? food or exercise or both? how do I change that?

    I slow down on my exercise and eat different things for about a week or so then I get back into it and I start to lose weight again.
