Ready to start again for the last time

I'm 37, married a beautiful woman and father of two children. I had open heart surgery to repair a congenital heart valve 6 years ago. Everything is going well in that department, thank God. But I've let my self go completely. I was at 277. I started the Biggest Loser diet and it worked really well - I was down to 250. But the website made it really hard to track my calories and workouts. Eventually I fell off the wagon and am back up to around 260 (weigh-in is tomorrow).
I really need to make a permanent change. That eating plan was really working for me - 3 meals and 2-3 snacks per day on a set calorie count, 4-3-2-1 food counts (servings of fruit/vegetables, lean meats, carbs, dairy respecitvely). I really need to add the exercise piece, though, which I didn't do last time.
Any other men on this site?
