I don't have large ones myself, but I had a good friend in college who had lots of back and shoulder pain from hers. It's not fun, but she eventually had breast reduction surgery -- when they're causing pain and back issues, doctors will often sign off on the surgery as "medically necessary," especially if you are having issues working out, already on the right path to health, and having the pain just walking around during your normal day.
After that surgery, she was much more comfortable doing everything she wanted to do.
And yeah, I think the boob-shrinkage is different for everyone. Mine always do (and there's not much there to lose), but some folks don't.

Yes they do seem to get in the way a lot, and I was told if they get any heavier I would HAVE to have had surgery, no ifs or buts about it. At the moment, just gonna wait til I get to my goal weight and see what they are like then.0
