Reverse Crunches and Arnold Chiari?

So I'm doing the 30 Day Shred and finding that standard crunches are fine, but when I do reverse crunches, I get an almighty head pain that feels like an icepick to the back of the head...
Now being an Arnold Chiari victim (!) I can relate well enough to the pain, but are there any other sufferers out there who are experiencing the same thing? Is it because of where the malformation sits? ie around the muscles that are obviously exercised during the reverse crunch? I'd love to hear your suggestions.
For the moment I'm filling in standard crunches for all the reverse crunches in the programme... However, would like to be able to do 30DS like a normal person  without the aid of daily codeine!
Thanks Y'All!
