Creamy Chicken Wild Rice Soup w/ pics and nutrition

I made this yesterday in the crockpot...it was delicious!!

8 oz. chicken breast, cooked and shredded
1 can cooked wild rice (equivilent to 2 cups)
1 medium onion
2 large carrots, sliced thinly
2 stalks celery, diced
3 chicken bullion cubes
1 can cream of potato soup
4 cups water
6 oz. fat free cream cheese
1/4 cup fat free sour cream
1 tsp garlic powder
1 tsp salt
1/2 tsp pepper
4 strips turkey back, optional

Combine all ingredients, besides the bacon in a crockpot. Set on high for about 5 hours, stiring often. If desired, cook bacon and crumble on top before serving. This will make roughly 10 cups of soup, depending on if you follow the recipe exactly.
