How do I start a "healthy living" plan?

Hi everyone! I am an on-again/off-again MFP user and I have recently fallen completely off the wagon. I am looking to get back on in a big way but I don't know where to begin. I was this to be a change for life but I am not easily motivated. There is only one week left of college before I graduate but I have been managing full time work and full time school for 4.5 years now and that has left very little "me time". Since my last final is next Wednesday, I feel like now is the best time to turn things around. Any suggestions on a good way to do this? Specifics are helpful. I just want a plan that will work and that I can stick to. Any and all input is appreciated Thanks!
