Heart Rate help

So I have noticed since I got a HRM that when I work out my HR tends to stick around 170, if I push the highest it has gone is 189 HOWEVER literally within 5 seconds of slowing or walking it drops to 160 (if I'm at 170 it can get down to 118 in the time it takes to tie my shoe) This makes me feel like I am not pushing myself hard enough even though around 190 is 25bpm above my range.. (124-167 i think) I can be out of breath, sweating, feeling like I cant do any more but 5 seconds later I feel like i didn't push myself nearly hard enough... (side note, I do the same thing with ST, say I do 20 pushups and can't squeeze one more out despite trying, I stop and feel like I haven't done anything until i start again)

Does anyone else have this problem?
