Athlete's Foot

It's not fun to admit, but I've got a case of Athlete's Foot, and it's driving me crazy. I don't know if I picked it up from somewhere or just developed it, but it's the first time I've ever had it.
Anyway, I've been using an OTC medicated cream, using vinegar/water foot soaks, keeping my feet as dry as possible, changing socks often, and even Tea Tree Oil to try to clear it up. While the itch has certainly faded, there are areas that are still raised and painful - particularly a blister-like spot on the end/ball of my second toe. It is in contact with the floor and the toe next to it constantly, and it HURTS!
Any tried and true methods for getting rid of this???

Keep putting the cream on.
You can put moleskin or a bandaid over it to keep it from rubbing the floor/sock/shoe.0
