I've fallen off the "weight wagon" and I can't get up

I have not had much success with weighloss at all, but I had weighloss surgery 1 1/2 years ago and I lost a total of 143 pounds. As of today I have gained 16 pounds back. Now someone may say that is not much, but when you've been overweight all of your life and had numerous health issues as a result even 1 pound matters.
Well this time around I want to lose the weight naturally and lose it the healthy way. So I need as much help and accountability as I can stand.
