How where to get professional help

My cousin needs to lose weight. She also has bad knees. Her insurance does not pay for dietician
so we plan to help her to get healthy.
She lives in portland, Oregon so what is your suggestion beside weight watchers. Is there any good weight loss clinics.
She needs someone to educate her how to lose weight and also meet her once or twice a month to make her accountible (sp?).
