How to maintain your losses?

I have lost around 100 lbs twice and 80 lbs as recently as last year. I always seem to slip back into my old habits and gain my weight back. I have already gained back about 20 lbs that I lost last year and I did not reach my goal to begin with. I joined MFP to help me actually hit my goal but I need to figure out how to keep the weight off.
I am pretty good at loosing the weight through weights, (during one of my big drops I was benching 250 5 times) and cardio (last year I ran a 10k in less than 1 hour). No real impressive feats but for me pretty good. Unfortenately I ALWAYS slip back and put the weight back on plus.
Any suggestions are greatly appreciated. My family history says I should not be heavy and the Yo-Yoing my weight does, does not help either.
