So what I want to work on is my back and the roll under my arm. Basically where your bra goes all the way around. What can I do at the gym? Or maybe outside the gym?

tricep dips, rows, tricep push ups, dead lifts (although that is lower back, but could only help), maybe some side bends or russian twist sit ups.

What is a russian twist sit up?

To build up the muscles you could do pushups, pullups, tricep dips, rows, or bench press. I'm sure there's more I'm not even thinking of.

To lose the fat over the muscles you'd just need to lose overall body fat.

It is a sit up, but when you are 'up' you 'punch' (I use 10lb weights) to each side. Left arm punches right and twist at the waist with the punch, then Right arm punches left and twist at the waist with the punch. My trainer recommends alternating arms each time you come up.

There's no such thing as spot reduction. Reduce bodyfat all over by eating at a reasonable calorie deficit for the amount you want to lose (not too much of a deficit), and regular exercise including strength training. It'll come off in time.
