Heart Rate Zone Questions (Insanity)

I'm about two weeks into Insanity and there's a lot of talk about heart rates involved. I'm having a really hard time deciding if I'm working "hard" enough to make the program effective. With regard to perceived exertion, I can't carry on a conversation (I can barely breathe!) and I push myself until I feel like my muscles might give out. But when I take my heart rate (I do it manually by counting beats on my throat) it's never higher than the mid 130s (typically 34-36 beats per 15 seconds).
My MHR is 190 (30 y.o. female) so this seems like it's not very high to me.
For Insanity, I'd like it to be HIIT, which means my HR should be up much higher, like 80 to 90 % of my MHR, right?
Is it possible that my heart is stronger than the rest of my muscles and I just can't get them to work hard enough yet? I'm getting frustrated and wonder if Insanity is really going to be effective if I can't get my HR to increase.
