motivated but not responsive...

Last year in the summer I used MFP everyday and for 8 months did great. When I went back to work (a teacher) I found that getting on here was so time consuming that I stopped every day... and then eventually stopped altogether. Having gained back 20 pounds I decided (with dr advice) I needed to try again seriously. For 3 weeks I've logged in, been doing exactly what the dr said, still struggling with daily exercise due to back and foot problems BUT try to do at least 15 mins a day, but I am NOT losing any weight :-( I have cut my calories to 1300-1400 a day (per dr instruction) and limited carbs (high blood sugar levels) so I'm confused why this worked so well last summer for me and now nothing... haven't lost even 1 pound :-( Anybody here with medical background that could offer some other reasons why I'm having difficulty?? I've battled my weight since the age of 16 and what I'm doing now has always been the trick for me to lose.... until back surgery I had been successful at keeping off 100 pounds for 12 years... so I'm not new to how to lose, maintain weight loss.
