HRM not reading correctly

I've had an HRM for several months now, in the beginning it was great and I felt like it was really accurate. However, recently it is alot less accurate, at times it will stop reading my heart rate entirely or says my heart rate is 60 when I'm at an intense point in my workout. Has anyone encountered this?? What am I doing wrong?

    Does it have a chest strap? If so, make sure the contacts are wet enough, and also that the strap is tight enough - if you've lost weight, you might need to snug it up a bit (but not too tight). Could also be time for a new battery - mine started giving me weird readings and cutting out - changed the chest strap battery and it worked fine again.
