Workout needed

Okay folks here's the deal. I'm not super good at thinking of what to do, and I'm pretty decent at following instructions. I really want to start working out effectively, but have no idea where to start. At home I'm following along with DVD's, but at the gym I just stick with the cardio because I know what to do (and they have the preprogrammed workouts already). I really want to get into the weight section, but I'm clueless as to where to begin.

So for those of you who enjoy working out/have a good grasp on what exercises are good can you design me a workout? I am looking for something to tone up my arms and shoulders/back. I do have access to a full gym. I normally go at night when they are unstaffed, so it's not like I can ask one of the workers there for help. I can youtube the exercises listed to get an idea of how they are performed, but right now I feel like if I were to try designing my workout it'd be like throwing spaghetti at a wall hoping something sticks/works.

Also, when lifting weights how do I know how much to lift? I don't want to injure myself, but I want to make it worth my time.
