Psychology of Weight Loss

I'm interested if anyone has read anything on what goes on MENTALLY when attempting a life change like we're attempting. Just today I read of a woman who had lost over 90 pounds and looked FABULOUS and she still only saw the flaws in the mirror. Another writer today said she seemed on the same cycle of success->self-sabotage->start again, and wondered if she didn't fear success. I've had a similar experience in my past where I finally achieved a loss of 30 pounds (bought myself flowers to celebrate), and instantly lost all motivation (even though I had another 50 pounds to go.)
Any insight on how long it takes for the mind to catch up with the body, or why we fail so much when we WANT it so much? ("Habit" seems a weak explanation.) What have you heard, or what have you experienced? I'm interested in success stories, too. How did your self-awareness, your view of yourself and who you were, change through the process?
