Q&A: How do I go about building muscle?

This is by no means a comprehensive writeup pertaining to muscle growth. However, in the context of the question I'm hoping it gives the novice reader food for thought. It can be very difficult when first starting out when you lack any real experience "under the bar." You're left either following some prepackaged program that may or may not fit your needs of trying to piece something together.
