Would I need a nutritionist, dietician or personal trainer?

Hi All!
I've been a yo-yo dieter for most of my adult life, with weight coming on easy and then coming off easy. But 2 years after losing an insane amount of weight, I was diagnosed with graves disease (an overactive thyroid condition) and after treatment, gained all the weight back plus some. Weight loss never quite been the same since. I did have a breakthrough last year in which intensive personal training and stress helped me lose 20lbs in 3 months before my wedding. I have since gained the weight back AND newlywed weight and am the biggest size I have ever been in my life.With MFP, its been 2 months and I've been able to take off 9 lbs on my own. But I feel I'm not being as effective as I can be with losing this weight and feel clueless and going in circles.

I'm looking for a professional (or professionals) who can customize my meals for me and customize a workout plan (ie 3 days cardio, 3 days strength training), vary the plans periodicly, track success, measure me, measure body fat, -- pretty much help me along because I'm sooo serious this time. I dont necessarily need someone to work-me-out in the gym for 2 reasons: Personal training is expensive and I don't want to pay as much as I did before for 3 days/wk PT and 2) I'm pretty sauvy around the gym floor, but I would need prior direction on what to do, for how long, etc.

SO! With that said, who would you recommend I consult? Do personal trainers help with caloric intake? Do nutritionists help with work-out plans? Have you heard of a personal trainers who do assessments/consultations and not necessarily actually training or am I floating on cloud 9?

If you were me (besides thanking God that you're now gorgeous and witty! lol), what would you do? Note, I am willling to pay for sporatic personal training session (bi-weekly or once-a month) and my insurance pays for a dietician/nutritionist. Any advice would help. And I sincerely that you for listening to my story and situation. Truly.

    In my experience I've been more successful with trainers and fitness experts who also do nutrition plans and counseling if you want the overall guidance of both opposed to just a nutritionist or two separate people. Most personal trainers also do nutrition counseling, so its about finding one in your area that does opposed to nutritionists who don't necessarily do fitness plans. It's definitely OK to ask them a lot of questions before choosing one that is right for you. They should all offer a free consultation. I use one who I think is great and she does do it for people across the country through email so if you wanted her info or her website to see more about her plans, message me.

    OHHH soo appreciative! So smart, Fitness expert who can provide nutrition counseling. And thank you for reminding me know that I can ask alot of questions before choosing, because I can be passive or feel obligated and you mentioned I am entitled to a free consultation with most trainers at gyms, yeah.
