Type 2 diabetic, insulin resistant and PCOS

Yea, I am a mess. lol I really am confused as to what I should/ shouldn't be eating. I have been trying the low carb diet and remaining active. I lost weight for the first two months quickly and it has slowed down to gaining back and fourth the same 5lbs for the last two months
I was reading about insulin resistant diets online and they suggest fruits and whole grains. Do I need to completely change my diet plan?????
