Lots of great ideas here .. have you ever asked her what is something she would love? Does she ever mention anything in conversation .. in passing .. "Oh.. I just saw the ads for that new iPad mini .. sure would love to check that out" .. or "I saw this cute outfit at Macy's (or Nordstrom)" ...
Pay attention and don't be shy to ask her. She may of course say .. "aww .. you don't need to get me anything.. " but I bet you can find her something super awesome and special .. something like a keepsake she'll remember forever ..
maybe create a photobook of the years you 2 have shared growing up and now .. :-)0 -
I second a gift certificate for something pampering. Or a season of a TV show she likes (but doesn't own), her favorite movie on DVD, a coupon to help her clean her classroom at the end of the school year...0

If she's a teacher I would say something relaxing/pampering like a massage or mani/pedi or facial. Or see if she needs anything for her classroom, my stepsister is a teacher and that makes her the most excited because then she doesn't have to spend her own money on her classroom.
Depending on how much you want to spend and her style you can get her a gift certificate to a clothing store, a nice wallet (Michael Kors, Coach, Kate Spade, Dooney & Bourke, Tory Burch, Hobo, etc) or a nice watch (Fossil, Michael Kors, etc) or a pair of earrings.
If she's into fitness what about a workout program she's really wanted or a gift certificate to somewhere like lululemon?0 -

Probably the best answer! I'm asking for a new toilet. I would love to get something pampering, but then I'd just want more and know that wont happen... so I just want things I need and that will make life easier.0 -

My brother and I are the same way. Last year he gave me a diamond tennis braclet which is gorgeous. Since she is a teacher and is probably low key with jewlery, you may want to to consider a pearls or opals. The mani/pedi is great and I am sure she would love it but it seems a bit impersonal Now, an entire spa day would be a different story. Good luck.0

I cheat - I ask other family members for ideas. For example, I asked my mom what to get for my brother, and she went off about how he needs a men's hygiene bag, because the last time he went to a convention, he just put his stuff in a gallon ziplock bag! So I got him a boring men's hygiene bag - then spiced it up with a Chocobo washcloth!0

Maybe a puppy that 's if she like dogs or some smell goods like Victoria Secret or Bath and Body works you can never go wrong there! maybe some earrings0

A weekend/night class - cooking, pottery, interpretive dance, etc
A magazine subscription
An ipod (if she lives under a rock and doesnt have one) with all her (or your) favourite music downloaded on it
Go to an artisan craft show and get her something beautiful for her home (wood/glass/stone art, a painting, or kitchen utensels), or a unique clothing item.
If she goes to the gym, paying for her membership for a few months, or some training/boxing sessions
If she likes rollercoasters, and you have a park near you, a day out there.
