Done it as best I can. Quite a few of the questions appear to be multiple choice (and say check all that apply), but use radio buttons instead of check boxes so you can only select one option. There's the same answer twice available on a few as well, and one of the questions says "Click here to enter a Question 8" or something along those lines. You might want to check over your survey again and fix the bugs.0
