Setting Realistic Goals....staying on a healthy path

Hello All!
I was reading an article earlier today that discussed how important it is to set realistic goals in regards to fitness and nutrition. The article talked about body image and how easy it is to get into the mindset that nothing is ever good enough. I think for those of us who lurk these forums regularly, it's clear that there are many that have unrealistic expectations.
How do you set your goals and stay realistic? Do you think that you put more pressure on yourself because of outside factors or find yourself comparing your success with others?
This topic is one that interests me greatly as I have found that since I started this journey in September 2011 I have gone through many phases that have bordered on obsessive. I am now at a point where I am more concerned with being healthy rather than skinny, but I won't lie, there are days when that view changes. There are still days where I struggle with my relationship with food and have difficulty acknowledging how far I really have come. I don't know if I will ever truly see myself as anything other than the "fat" girl I used to be.
