Myelofibrosis and decreased fitness

My mother suffers from myelofibrosis and is seeing her fitness decrease significantly. She is having trouble walking, often tired as the disease advances. I was wondering if others here have experience with this disease. I am quite aware of the hematological function and the fatigue that can occur with low blood counts but we are seeing the classical "MPD Fatigue Effect".

Over the next days, I'm going to see if I can help my parents define a fitness / nutrition program for her, to deal as well as she can with this degenerative disease.

Any input by people who have dealt with this disease or other functional muscular decrease or fatigue syndromes would be appreciated.
