What makes you fall off the wagon?

So, what makes you "fall off the wagon" of good eating? And how do you prevent it?

For me, it seems like every week or every few weeks I get to a point when I'm just so hungry.....then I relax, and the next thing I know I'm eating everything I swore I wouldn't touch. What can I do to prevent this from happening?
