Dieticians out there here's a bit of a challenge!

I have a condition called 'Dumping Syndrome'. I've had it for decades but my Doctor refuses to refer me to a dietician. She seems to think she can deal with it herself but she doesn't really understand the problem. Obviously this doesn't help me and I will be switching Doctors in the near future but in the meantime here are my bizarre dietary needs:
1. I need to eat five to six times a day and meals need to be around 250 to 300 calories - no more
2. In any one meal carbohydrate must be less than half of the meal. Fat content can go up to compensate to some extent because while I can't tolerate carbohydrate (which is the real problem here especially simple sugars and the like) I can tolerate extra fat
3. Also there is a danger of accumulation of carbohydrates over a number of meals so no more than 30% of the total in any given day can be carbohydrate.
4. I can eat small quantities of potatoes, pasta and so on and rice seems to be generally ok so long as they don't accumulate over the day as in 3 above. The real danger stuff is the hidden and added sugars.
5. I don't eat much red meat as it tends to bung me up but chicken, fish and so on are ok but again in small quantities.
6. I take a high dose RAW probiotic, prebiotic and vitamin pill each day which helps to firm up stools (this is gross but without that I can literally excrete whole undigested food which isn't very pleasant!) and to help ensure I get the vitamins I need. I also take a high fibre pill derived from fruit extracts to help slow down the digestive process.
7. I can't use protein shakes to supplement because the generally available ones available in the uk seem to have sugars in them or use milk which would have much the same effect. If anyone knows of supplements that are ok or alternatives to cows milk I would be interested to know about them.
I need a diet plan of about 24 of these small meals to see me over a few days.
