    Eat less, move more.

ETA: If you need more specific tips, you need to be more specific. What do you do now, what are your struggles?

When it comes to weight, eat less calories. This can mean eating calorie dense food in small servings, or eating less calorie dense food, in larger servings. As long as the calories are the same (and less than what you are eating now) you will lose weight.
When it comes to health, it is a different story. In general, most drs will tell you that the more you can cook from scratch, and the more you limit "junk" food, the better for you. You can start here http://www.hsph.harvard.edu/nutritionsource/healthy-eating-plate/ if you need some rough guidelines, although obviously variations exist.
And move. It does not really matter how you move, just move. Go for a walk, swim, use a bike, go to the gym, put on some music and dance, as long as you are moving and can commit to it, you are on the right track :)

    This!

I will add that the best tip I can give to someone starting out is log EVERY SINGLE THING YOU EAT! There are no free calories and if you log everything, you will soon start to see patterns about what foods fill you up, help you stay within your calorie goal and which foods do not. The scale did not really start to move for me until I did that.

If you struggle with what to eat, find some friends with open diaries or search the forums here. There are so many wonderful tips I learned just from doing this and that really helps because this is real people who are being successful.

    Good luck!

Plan ahead so if you find yourself in a position of having to grab something on the run you have healthy things available and you won't have to stop at a fast food place because your hungry.

Yes, this! For me, that's a huge part of the whole "eating less" thing. My first week, I told myself I was planning ahead, but I'd often log my food after eating it, and guess what? A lot of the time the calories were higher than I expected! Plan ahead, log in advance. And don't beat yourself up when unexpected stuff happens and your plans get messed up. It happens, so make sure you're as prepared as you can be.

    You can do it! :)
