Why am I failing all the time?

I know why I fail it is because I overthink too much. I am now over 400 pounds and noticing things going on with my body that are new. My knees hurt ofter, back often and many more things. I do have high blood pressure and am now on meds for it.
I lost weight once before I bought a scale out of curiosity and was 371.2 pounds the next day I said time to lose weight I am too young to die I have 4 kids and 3 grandkids that I need to be there for.
I started weighing my food spend $100s on exercise equipment and lost 50 pounds in 100 days but during the process I hurt my neck lifting weights so I sold the gear and went back to my old eating habits for some reason to this day still not sure why.
So 3 years later and 100 pounds more I can not seem to get going to lose weight. I know how as I have done it before. I think it is because I am overthinking it or have come to terms that I will die due to my weight.
I overthink because I think right from the start that I need 30+ grams fiber and so many fruit and a certain amount of cups if veggies that I get so over whelmed that I do not eat anything healthy and eat same garbage.
Now I know that lots fiber, fruit and veggies are healthy but I also know that I just need to eat fewer calories now to get some weight off me and when I lose 100 pounds or so then start thinking add all the veggies and fruit in my diet.
I guess I am just looking for members who were like me if any were could share there story on how they finally started because I need this weight off quick as I do not and can not die young.

You're failing because the things you're doing aren't sustainable. You're also giving up in the face of adversity.
If your diet isn't sustainable, you're not going to keep with it. Maybe instead of worrying about the little things just log the foods you do eat (and stop labeling them as garbage) and fitting them into your calorie goal that MFP gives you. Instead of giving up when you injure yourself, allow yourself to heal, continue logging and then get back to it.

Start by just eating less of what you currently are. Don't make drastic life changes yet.0

Instead of quitting when you don't see results - keep going until you do see results.
Don't over think everything. Log what you do eat, see where your weaknesses are and just eat less of the things you like. You're never going to stick long term to a diet filled with foods you don't enjoy.
Don't put a time limit on your weight loss, and make small changes to your lifestyle, one at a time. For example, try making a new habit out of drinking 8 glasses of water a day. Once you've got that down after a few weeks, move on to a new habit, like maybe adding more veggies to your meal (veggies you like mind you) and so on.
Don't focus so much on it, plan your menu out on Myfitnesspal and move on with other things instead of obsessing over food all day. You should only think about food when it's time to eat.0 -
Redefine success and failure! If you start living a lifestyle to be around for your kids and grandkids, you are successful. The weight loss, the energy, and the longevity of life are symptoms of that success, not the success itself. Set MFP with a conservative weight loss goal and log and live baby!!! If you fall off, own it, record it and move on. Lean on you friends (MFP and real world) and most importantly believe in yourself for the long term, not this week's weigh in, or next week's one day of bad eating.
You can do it and feel free to add me if you'd like motivation along the way. I'm on a path to lose 125+ myself.

I understand the snowball that weight gain can become, it's hard, but as you stated above, you have a lot to live for and as someone who recently lost her mother early (she was 65, I'm 28) due to complications from conditions that were exasperated by her weight, I can tell you there's nothing more precious than your health.
If you want to be there for your children and grandchildren in a tangible way, you can't give up, and if you stick with us here on MFP you won't want to.
There's so much support here if you want/need it.
You don't have to restrict yourself or make it seem impossible, if that's how you go in, of course you're going to fail. You have to make small changes, and maybe the first one can just be logging everything, even if you're going over, just get into the habit, and things will start to make sense, and look different.

One you may want to explore is why have you may have sabatoged your self when you see success. Are you afraid of attention or feel you are not worth it. Those voices are worse than the food we eat. You can do it and you are worth it0

Start small and build.
Cut one bad habit and replace it with a good one. When that new habit becomes commonplace, replace another bad habit with a good one. Every little thing you do will make a difference.
And with exercise, yah, it's really easy to do too much right off the bad, especially with weight training where good form is absolutely essential. Start small- an extra 20 minute walk every day.
Be kind to yourself! This is a huge journey! And what @Pinkylee77 said- Listen to yourself, to your negative fears and self-doubts, but argue with them. Warn them that they can't stop you! That you are invincible.0 -
When i was about 24, i went from being 200 to being 170, but i wound up getting really turned off exercise when i hit 170 because I wound up paying for a few sessions with "The good" personal trainer at my local gym, and wound up being physically ill during one of our training sessions, because he was pushing me that hard. I felt terrible and i never went back (think i still have 4 session paid for at that gym actually!)
Anyways, I tried going to the gym a few times since then, and have always hated it. My weight has bounced around between 160 and 180 since then. Only lately have i started pushing a bit harder on the exercise front again, but at my pace now... mostly I walk and do yoga, and i'm trying to line up a bicycle for summer, and work up to walking to a little bit of running. And i take it to the point where i work up a good sweat, and start huffing and puffing a bit, and that's where i leave it. Good enough.
My whole point, after all that rambling, is that you can get turned off exercise and dieting when you push it to the extreme, and you should go at a pace that makes you lose without losing patience. exercise and fitness should make you feel better not worse.0 -
You can lose weight by eating what you want and by paying attention to calories one day at a time.

I started around 320 this time, and have lost about 80 lb so far. Like you know, you don't need to eat fiber or fruit or vegetables. My own success-story-in-progress probably gets a subtitle of "How to Eat Crap and Lose Weight Anyway".
Some things I do:
1. I pre-plan my meals the night before.
2. Focus on the wildly important goals for your own health. For me, there's one critical goal (calories) and one goal I try to hit if I can (protein). For you, those might be different because your health needs are different. But again, focus only on the wildly important. You shouldn't have more than three wildly important goals (if everything is important, nothing is important).
3. A multivitamin is my insurance for micronutrients.
4. I plan my days around one very filling meal, and work from there.
5. I do exercise, but this is for health and not for weight loss.
6. In the event my workout generates not just soreness but injury, I take time off and let myself heal.0
