I dont feel hungry and am losing weight about 20lbs in 6 weeks but have a lot to lose I just don't know how much sugar and carbs I should eat since for the first time ever being tested for sugar it was high at 116 a few hours after dinner doctor just said watch sugar what does that really mean

Why don't you ask him/her? It doesn't make any sense to tell you something like that without giving you some guidance.

I go to a VA hospital if that tells you anything and shes not a doctor I know very well it was my first visit with her

What does this even mean? You keep referring to her as a doctor but now you say she's not a doctor?

If you don't like your doctor (or whatever she is), ask for a referral to a dietitian.

I'm with the VA too. Sometimes it takes a little more work to get what I need. On the plus side, they don't limit my visits to a mere 10 or 15 minutes.

Try the VA TeleMOVE program. I found it helpful, and could have had consultations with a dietitian if I wanted, which I didn't, but I think could be helpful for you.

I took it to mean that, being her first visit with this doctor, she’s unfamiliar with how she practices

She is a doctor but is new there and that was my first visit with her. I had a doctor that I really liked but she quit after 5 yrs I could talk to her so easy but since she left they cant keep a doctor there. I will ask her about the sugar and ask to see the dietitian. It may have been a 1 time rise and I have lost 20lbs since then and dont eat junk food like all thoses sweet cakes.

I looked at your diary, and your calories are shockingly low and you aren't tracking sugar/carbs.

I think you are talking about the pie chart, and that's meaningless, it should be ignored. Percentage of carbs from some arbitrary number of calories does not matter. What matters is hitting your goals for TOTAL GRAMS of protein and (to a lesser extent) fat, and carb grams don't matter unless you are diabetic or you are eating too many to hit your protein and fat targets.

My brief look suggested that your protein and fat numbers might be a bit low, but because overall calories are low.

I'd pick gram goals, watch that, make sure you eat vegetables as well as fruit, and ignore sugar and watch fiber instead -- fiber is a minimum -- or, in the alternative, ignore sugar from whole foods and just make sure you eat sweets in no more than moderation, which you say you already do, so great.

For concerns about IR/T2D, you'd want to make sure carbs are eaten with protein and fat and that it has some fiber, so maybe have some nuts with your grapes or eat them at mealtime with other foods.
