Hello everyone. I have tried to change my lifestyle and making healthy choices. However, I have always failed to stick to it. I lose interest and gain back all the pounds I lost. Any tips on how to make a change last forever? Thanks

Hi there. Welcome. There are lots of great resources to help you on your way. Check out the stickies (most helpful posts) at the top of the forum boards. Great, to-the-point info to help you cut through the BS and avoid many of the pitfalls people make when they first start out. Helped me a lot when I first logged on to MFP. Here are three of my favorites:

You probably have many small habits that you need to change.
What has worked for me is tackling problem habits one at a time. I make one change the focus. I focus on making that one small change every day, and give myself time to get reliable for that new behavior (think 3-4 weeks).
Once I am reliable for something, I then tackle a new small habit.
On a related note, don't make the mistake of thinking that you have to feel like it in order to do the thing. You can feel like doing something else and then choose the action you committed yourself to.
One of my favorite writers/speakers on this topic is James Clear:

Welcome to the community!

I agree, start with something small. My first thing was just filling out the food diary. That takes a little time to get used to.
As I went along, I started changing my foods. I used to be addicted to gumdrops and had them every night. I replaced those with a healthier snack. Then I didn't even want that anymore. Now, I don't snack at night.
Now, I eat mostly plant based with fish, eggs and dairy. Oh, I joined almost a month ago.
You can do this. You just have to want it bad enough. ☺3 -
Get into the habit of filling out your food diary. Even if you go over your calorie goal, you need to log everything you eat so you can establish the habit of logging and learn how not to overeat in the future. A lot of people pre-log everything they plan to eat for the day so they know how they'll be spending that day's calories. Set a reminder for yourself to log your food, whether that's pre-logging the night before or first thing in the morning, or logging at each mealtime.5

I actually log a couple of days ahead. It means less stress and more accountability for me. ☺1
