She's just told me that her husband's work shift has changed which means that he can't go to the hospital with her (he'll be working 8am-4pm so it's not possible to get to the hospital during clinic hours) so she cancelled the appointment 🤬😵🤬1 -

Just so you know... that's part of the nature of the beast of the disorder. Unless someone physically walks her there -- even without an appointment -- she's going to keep cancelling. Anorexia is an egosyntonic disorder, which means to *her*, this feels OK, even with all of the health side effects. She's not going to go to an appointment on her own, because the disorder is telling her that losing weight feels good, and getting help is bad.
Someone needs to take her phone, and needs to literally walk her to the hospital/the emergency room, and not leave until she's admitted. She's emaciated, and at this rate, she's going to kill herself.12 -

Reminds me of when my housemate needed to go to detox. Her sister gave her a choice: "You can drive with me, or I am calling an ambulance and they will take you and you will get billed for that."
My housemate tried to delay ("I need to take a shower, I need this, I need that"), but her sister, who had been through this before, held firm and we got her into detox.12 -
Hi, I just heard from the OP and she asked me to post another update.
She had an appointment with the hospital MH team on Monday. Unfortunately there is no specialist ED help in her province so the psychiatrist gave her a couple of doctors names who do specialise - 1 is in İstanbul and the other is in Izmir, both too far away for her to go to as an outpatient (if that were an option) and she outright refuses to be admitted anywhere, near or far, as an inpatient.
So, a compromise was reached; she'll start a mix of fluoxetine and another drug (she couldn't remember the name). Apparently this combination is widely used in the US to treat anorexia. She'll give it a try for a month or so and then reassess.
She went for blood and unrine tests today and is waiting for a cardiology appointment.
She'll update again once the results are back.11 -

I am still on the extremely concerned side of things as, by the looks of it, drug based treatments are considered a support as opposed to main method of treating this (apparent) type of ed.

The solution for treating anorexia (which it sounds like this is) is to gain weight. Bottom line. There is no medication that will treat the underlying problem and medications are really only to support someone with regaining weight. The best success in the treatment of eating disorders like this are to regain a normal BMI, and it sounds like she really needs to be inpatient to help her do that.
When you have such severe caloric restriction--it may even be dangerous to increase back to a normal calorie intake without monitoring (with labs) because it can so drastically effect your electrolytes because of referring syndrome.
This kind of sustained very low-calorie diet in someone who is already malnourished with low BMI is very, very dangerous.
Anorexia is a deadly disease and is among the most serious mental health conditions with the highest mortality rates because of this.
Does her husband recognize the severity of her disease? Could her dad come back? It sounds like she needs to have all her family/friends mobilize to support her getting into formal treatment, even if that means she has to go back to access NHS services.4 -

Actually, the best way to treat an eating disorder is to treat what is causing it in the first place. It isn't as simple as eating more when there are various factors for not eating enough in the first place. Anorexia is complex but a combination of genetics, psychological factors and cultural influences can all come into play. Medication can be of help for much more than with physically regaining weight. It, combined with therapy, can also be of assistance to any other psychological factor which comes into play.8 -
In the uk the NICE treatment protocol for anorexia is some weight gain first. Therapy as well. And also medication in some cases. All monitored closely by a multidisciplinary treatment team.
OP obviously won’t get this in turkey. But maybe just maybe she will feel a bit better on the meds and this may lead to her rethinking her plans and getting the full treatment package by returning to the uk5 -

Bingo. Starvation and severe underweight status seriously affects the brain. So yes, gaining weight won't solve the problem, *but* it at least works to try to remove one of the noise signals that affects recovery.
Just like eating more won't fix the root cause, all the talk therapy in the world won't fix the health issues and downstream brain effects that having a BMI in the low teens causes. Hopefully Sefa has people working with her who will step in and remove those barriers. She's not her best advocate right now.2
